On 5 April 2017, a General Dynamics F-16C Fighting Falcon (87-0306) operated by the 113th Wing, Air National Guard, USAF, crashed shortly after takeoff from Joint Base Andrews (ADW/KADW) in Prince George's County, Clinton, Maryland. During the initial climb, the aircraft suffered a loss of engine thrust followed by an engine fire, forcing the pilot to eject safely. The aircraft was destroyed, but there were no fatalities.
The USAF Accident Investigation Board determined that the accident was caused by missing components inside the aircraft's main engine control (MEC). Specifically, the MEC lacked the required 600-degree training ring and anti-rotation pin in the differential pressure pilot valve. This defect allowed excessive fuel to enter the engine, resulting in severe engine overspeed, extreme over-temperature, an engine fire, and catastrophic engine failure, making continued flight impossible.
