Hepatocellular Carcinoma: Sorafenib is used to treat unresectable hepatocellular carcinoma in patients (HCC). Renal Cell Carcinoma: Sorafenib is a drug that is used to treat individuals who have advanced renal cell carcinoma (RCC). Pharmacology In vitro, sorafenib is a kinase inhibitor that inhibits tumor cell growth. Multiple intracellular (CRAF, BRAF, and mutant BRAF) and cell surface kinases (KIT, FLT-3, RET, VEGFR-1, VEGFR-2, VEGFR-3, and PDGFR-ß) have been found to be inhibited by sorafenib. The signaling, angiogenesis, and death of tumor cells are considered to be mediated by many of these kinases. In immunocompromised mice, sorafenib reduced tumor development and angiogenesis in human hepatocellular carcinoma, renal cell carcinoma, and numerous other human tumor xenografts. Dosage & Administration Advanced renal cell carcinoma: 400 mg bid. May continue until patient is no longer responding or unacceptable toxicity occurs. Hepatic Impairment: No safety data is available for use in patients with Child-Pugh C hepatic impairment. Should be taken on an empty stomach. May be taken with a low or moderate fat meal. If the patient intends to have a high fat meal, sorafenib should be taken on an empty stomach at least 1 hr before or 2 hr after meals. Swallow whole, do not chew/crush. Interaction Carbamazepine, dexamethasone, phenobarbital, phenytoin, and rifampicin are all CYP3A4 inducers that can lower sorafenib plasma levels. Coadministering doxorubicin and irinotecan with sorafenib may raise doxorubicin and irinotecan plasma concentrations. Contraindications Patients who have a known severe hypersensitivity to sorafenib or any other component of Sorafenib should not take it. In individuals with squamous cell lung cancer, sorafenib in conjunction with carboplatin and paclitaxel is contraindicated.
